You can set it up with TOTP, that is the 6 digit code that changes in the app - OnlyKey User's Guide | Docs
For push authentication where the app prompts to approve login you can only use Microsoft Authenticator app for that.

Setting with TOTP: I believe I would have to delete all the accounts on the Microsoft Authenticator,
and enable them again so I can see the code?

Yes, you set up TOTP on phone or OnlyKey once. To set it up again on OnlyKey you have to go through set up of new TOTP 2FA account.
